Age in Place
Many senior living facilities do not have the capabilities to adjust their care when a resident’s health changes or declines, and we know how frustrating it can be to have to move in and out of different buildings. Our levels of care allow us to adjust care plans as medical needs change to provide the best opportunity to stay with us, without having to change buildings.
Age Together
Many couples who have spent their entire lives together may start to experience different needs in the late stages of life. With our care model, they can continue making memories together and receive individualized levels of care, all under one roof.
Care Choices
At Shelby Gardens, we offer a full range of senior care options designed to support independence today while adapting as needs change. From assisted living and memory care to short-term respite stays, our personalized approach ensures your loved one receives the right level of care at every stage.

Standard of care
We only hire people that have a passion for caring for others. We are dedicated to providing a safe, and meaningful environment where your loved one will always feel like the staff that is taking care of them are extended members of their own family. Our highly-trained staff provides individualized care plans that balance physical well-being, safety and overall health while encouraging the ability to maintain one’s independence.
Licensed nursing professionals on-site. Many other facilities will only have nurses on call. We keep a certified nursing professional on-site at all times to ensure that should a medical need arise, we are able to address it at any hour of the day or night.
Routines are a staple of a healthy life, and we want to ensure our residents can continue the ones they had prior to moving in with us, which is why residents can choose to keep their personal physician, if they so please.
We offer services like a mobile podiatrist and more to make our residents’ lives as convenient as possible.
Expert oversight from our Director of Quality Assurance, Joann Kulniszewski, and her team working to promote the best possible resident outcomes.
Medical Partnership
By our Medical Directors, Dr Christen Fleming
Dr. Fleming was born in Texas, raised in a suburb of Chattanooga and moved to Knoxville as a college freshman. With family in the area, she fell in love with the city (and her husband!) and knew she wouldn’t leave Knoxville for long. She completed undergrad at UT in finance and small business management. She then attended medical school at UT Health Sciences Center in Memphis and completed residency in Internal Medicine at UT Medical Center in Knoxville.
Dr. Fleming practiced primary care in an outpatient setting for 5 years before stepping back to spend time with her 3 beautiful children and husband. She loves the long term relationships she forms with patients in a primary care setting and the satisfaction of improving patients’ health over time. Dr. Fleming loves teaching patients, friends and family alike about how to improve their health and reach their medical goals.
In her free time, Dr. Fleming loves to travel, volunteer at her church and her children’s schools, spend time with family and friends, watch her children’s and UT’s sports, and explore our beautiful city/region.
At Shelby Gardens, our residents have the opportunity to keep their current physician, but if they wish to switch to our medical directors, they will gain the added benefit of Telemedicine which includes:
• Access to a doctor without leaving the comforts of our community
• Reduction of unnecessary emergency room visits
Common Questions
The right level of care depends on your loved one’s daily needs, safety, and independence. Our team helps families assess current needs and recommends a personalized care option that offers the right balance of support and independence.
It’s often best to begin exploring senior care before care needs become urgent. Planning early allows families more choices and peace of mind when making important decisions.
Assisted living provides support with daily activities, while enhanced assisted living offers additional hands-on care for residents with higher medical or personal care needs. Both options are designed to promote comfort, dignity, and safety.
If care needs change, we adjust care plans to provide additional support while allowing residents to remain in a familiar environment. This flexibility helps residents age in place with comfort and confidence.
Choosing senior care can feel overwhelming, especially when needs are changing. Our experienced team is here to answer questions, explain your options, and help you find the best path forward for your loved one with confidence and peace of mind.
